The new MyHQ247 Patient Portal

Patient on computer using patient portalWe make keeping track of your health easy. Health Quest now makes it effortless to securely access your medical records through one patient portal, MyHQ247. The newly enhanced MyHQ247 provides secure online access to your health records for all Health Quest sites of care, including: our four hospitals (Vassar Brothers Medical Center, Putnam Hospital Center, Northern Dutchess Hospital, Sharon Hospital); The Heart Center; Health Quest Medical Practice, P.C.; and Sharon Hospital Medical Practice.

Through the enhanced MyHQ247, you can:

  • View lab and radiology test results.
  • Securely message your outpatient providers.
  • View notes from your visits.
  • Request prescription renewals.
  • View vaccination history.
  • Pay your bill online.

Patient Privacy and Security

Your health and financial data is stored using top encryption and security technology to protect your information. Patient information is always encrypted, firewall-protected and is under surveillance 24/7.

If you wish to enroll online, you must have provided your e-mail address to a registration representative at a Health Quest hospital, HQMP office or Heart Center office.


Note: MyHQ247 allows for self-enrollment however; your email address must be on record in the Cerner system (new for the HQMP and Heart Center practices) or you must use your medical record number which can be found on discharge/visit summaries and patient ID bands.

Enhancing Your Care

During our transition to the new MyHQ247 portal during Spring/Summer 2019, many of our HQMP and Heart Center offices will be adapting to the new system. You'll notice some changes to make your care more accessible and convenient. As our providers and staff adapt to the new system, you may notice longer wait or appointment times. Your HQMP or Heart Center provider is transitioning to the new system as part of several "waves" taking place this Spring/Summer. Note the chart here to check if your provider's office is on the newly enhanced portal.

  • If your provider's office has already transitioned to the new MYHQ247 patient portal, follow the directions above to access MyHQ247.
  • If your provider's office has not yet transitioned, please find the links below for the old HQMP and Heart Center patient portals(s).
  • If you have technical questions with accessing your portal, please contact support at (877) 621-8014. The support team is available 24/7.

RelayHealth/Sharon Hospital

As of May 1, 2018, Sharon Hospital joined Health Quest in implementing the Cerner electronic medical record solution. If you are looking to view your old patient portal through RelayHealth, you can access via the link below. Please note, no new information from your visits or tests after May 1, 2018 will be available in the old RelayHealth portal.
