Hospital Patient Portal

What if I have questions or trouble accessing my information on the hospital portal?

Where does my health information come from?
Your health information is pulled directly from your Health Quest's electronic medical record in Cerner.

How do I view my health information?
On the navigation bar on the left, click the My Medical Record section. This will expand the My Medical Record module and allow you to select View Clinical Record Summary or Lab Results. Medications and Allergies show only currently active information — they do not display past medications or allergies.

What is a Continuity of Care Document (CCD)?
The Continuity of Care Document (CCD) is an electronic document exchange standard for sharing patient summary information. Summaries include the most commonly needed information about current and past health status in a form that can be shared by all computer applications. You can view or download a summary of care from your medical record as a CCD in the Download Copy of Record section. It provides a snapshot in time of specific medical information that can be exported to another medical record system.

What is contained within the Discharge Information section?
The Discharge information section includes any educational documents that were provided to you at the time of discharge, as well as any discharge summaries from your visit. You must have a PDF reader to view or download a document.

How do I change my password?
In the upper right corner of the screen, click on the Account Settings link. To change your password, click Change Settings. You will be asked to enter your current password and then your new password.

HQMP & Urgent Care

How is the information transmitted?
Your health information is pulled directly from HQMP electronic medical record system, eCW.

Will everyone be able to access my information?
No.  The information is securely transmitted to your patient portal account that only you have access to. You are provided a unique username and password via email. No one but you has access.

Will my personal Health Information be sent to my email?
You will receive an email stating PHI information is available on the portal, you will have to log in with your credentials to review this information.

What if I send an emergency message to the doctor and they do not respond right away?
The portal should never be used for urgent or emergency situations.

If I make an appointment via the Patient Portal, how will I know that I have one?
You will be requesting the appointment; someone from the office will be reviewing those requests daily and getting back to you within 3 business days with confirmation or alternate options.

If I check the box to have related health information and events sent to my email, will I be receiving all kinds of junk mail and solicitations?
If you choose to have related health information and events sent to your email, the only information you receive will be from Health Quest. No information will be sold or distributed to any other organizations.

The Heart Center Patient Portal

Where does my health information come from?
Your health information is pulled directly from The Heart Center's electronic medical record system, AllScripts.

How do I view my health information?
On the patient portal, click on the "My Health" tab and it will display the following information:

  • Diagnoses
  • Surgical history
  • Medications
  • Vitals
  • Allergies
  • My care team

How long will it take a Provider or Clinician to get back to me when sending a secure message from the patient portal?
Someone from the office will respond within 24 hours.

Will everyone be allowed to access my information?
No. The information is securely transmitted to the patient portal "Follow My Health". Only you and your proxy will have access. You have a unique username and password, which no one has access to.

What information will I have access to on patient portal?

  • Past and future appointments

How do I change my password?
In the upper right corner of the screen, click on the "My Account" tab. In the drop down box, please click on "Preferences". At the FMH Secure Login, please click on "FMH Secure Login Preferences" box shaded in blue. Enter your current password and then enter your new password twice to confirm. Then click on change password.
